Notice to Employees Regarding Form 1095-C – 2025 Tax Year
The City of Two Rivers provides this notice in accordance with Internal Revenue Service (IRS) regulations under the Affordable Care Act (ACA).
As an Applicable Large Employer (ALE), the City is permitted to furnish Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, upon request rather than automatically mailing the form to all full-time employees and other individuals entitled to receive a statement.
Form 1095-C provides information about health coverage offered to you by the City during the 2025 calendar year. You may not need Form 1095-C to file your federal income tax return; however, the information on the form may assist you in preparing your return or for your personal records.

Requests may be submitted in writing, by phone, or by email.
Timing of Furnishing the Form
Upon receipt of a request, the City will furnish Form 1095-C no later than March 2, 2026, or within 30 days after the date the request is received, whichever is later.
The form may be furnished by mail or electronically if the individual consents to electronic delivery.
Notice Availability:
This notice will remain accessible on this website through October 15 of the year following the applicable calendar year.
